Investigators Cite Human Error in Blue Rhino Fireball June 4, 2014 | WMFE, Orlando - Investigators blame human error for a massive series of explosions last July at a Tavares propane plant. Nine people were injured, and residents for miles were rattled. [Photo: The fireball launched debris outside of the plant. By Amy Green] A report from the state Department of Agriculture concludes workers at the Blue Rhino plant were venting propane tanks when someone started a forklift. The report says the workers failed to vent the tanks under conditions that would allow for the vapors to be dispersed. Tens of thousands of 20-pound propane tanks, the kind used for grilling, exploded. Three 30-thousand-pound storage containers were not damaged. Already Blue Rhino faces federal fines for the incident. |
